Home Care - Cognition Screening

Home care organisations helps people with more or less daily routins as mediciation, hygiene, cleaning. The daily routins can be completed with a regularly cognition screening.

The home care organisations have their planningtool/to-do-lists with the daily routins for each customer in their mobilephone and a screeningtool can be built into the planningtool. If a signal is captured in the screeningtool it can be presented in a report. A "customerreport" with repeated signals can be sent to responsible health care organisation.

(About 165.000 persons >65 years are granted home care in Sweden. There are similar projects with screening of drug side effects in elderly.)

  • I like this! I am working on a solution for person-centered dementia home care, too, and have heard several times that it is the home care staff that notice cognitive problems in their clients first. The challenge in home care is that they have very limited time to perform their tasks and they are only paid for the tasks comissioned by the care manager (biståndshandläggare). Could a separate "order" from a care manager be part of a standard care procedure for every person over 65? Say, an additional 30 minutes once every couple of months to perform screening? How could we make it happen?
